Short-Term Disability
This is income replacement (60% of your base pay) should you become unable to work for a period of time due to an illness or injury.
Long-Term Disability
This provides income replacement (60% of your base pay) after you have been disabled for 180 days.
Basic Life And AD&D Insurance
UF Health provides you with one times your base pay for Life Insurance and AD&D. Basic Life Insurance provides benefits to your beneficiary if you die.
Supplemental Life and AD&D Insurance
You can elect up to three additional times your base pay through this plan. Supplemental Life and AD&D would pay benefits to your beneficiary upon death.
*Evidence of Insurability
If you are wanting to enroll in Short-Term Disability or Supplemental Life outside of your initial eligibility period, you will have to complete Evidence of Insurability and be approved.
*Travel Assistance
This assistance helps employees and their families prepare for trips and during critical situations while away from home. It can assist with finding qualified medical providers, legal services, or with replacing lost credit cards and passports.
