Life Insurance provides benefits to your named beneficiary upon your death. If you die as a result of a covered accident, AD&D (Accidental Death & Dismemberment) pays your beneficiary an additional amount equal to your Life Insurance.
AD&D may also pay a benefit to you directly for other covered losses as a result of a covered accident, such as if you lose a hand or foot or suffer paralysis.
Full- or part-time employees who work 20 hours per week (.5 FTE) or more are automatically covered at one time their base pay.

Employees should identify who they want to receive these benefits. Beneficiaries can be changed at any time during the year.
Email the completed Beneficiary Designation form to mybenefits@jax.ufl.edu, and the Benefits Department will make the changes in PeopleSoft.
