Personal Leave of Absence (LOA)

Employees who are either not eligible to take FMLA or have exhausted all of their FMLA time may be eligible to take a Personal Leave of Absence (LOA). Employees should verify with The Standard that they are not eligible for FMLA prior to requesting a Personal LOA.

Eligibility

All employees are eligible to request a personal leave of absence.

For employees with less than one year of service, personal leave will not be approved for the following reasons:

  • Elective medical procedures
  • Care for a family member
  • Other nonmedical leaves (i.e., educational)

Guidelines

  • Only for continuous time off
  • Up to 90 days
  • Only one LOA per rolling calendar year
  • Not Job Protected

Subject to approval based on:

  • LOA reason
  • Circumstances
  • Needs of the business

How Do I Apply?

Requests should be submitted 30 days in advance when possible.

Medical Related Leaves

If the leave requested is for medical reasons, Medical Support forms will only be approved if signed by a medical physician (APRN or other nonmedical or non-DO’s will not be approved)
