Qualifying Life Events
Some examples of qualifying life events are:
Birth / Adoption
Marriage / Divorce
Gaining / Losing coverage
*Supporting Documentation
You will need to provide documentation that supports your life event. Changes to your coverage will be effective as of the date of the event.
*Dependent Documentation
You will need to provide supporting dependent documentation when adding dependents who are not currently covered under your medical, dental, or vision coverage.
*How Do I Make Changes
You can initiate a Life Event through Self-Service for most qualifying events.
You can also send the Supporting Documentation to Benefits at mybenefits@jax.ufl.edu, and they will help make the changes in Self-Service.
* How Do I Change My Name
You can request a name change by completing the Name Change form and submitting it along with a copy of your updated social security card to HRIS@jax.ufl.edu.
