Professional Licensure, Certifications, and Registrations (HR-02-003)

What happens if I do not meet/adhere to the 72-hour deadline before my certification/licenses expire?

Failure to renew 72 hours (3 days) prior to expiration date will result in suspension up to 30 days. Employees with invalid, revoked, or expired licensure will not be permitted to work at UF Health Jacksonville.

What documents are required for certification/license renewal verification?

Certification/Licensure must be from the original issuing source of certification/licensure, such as Florida Department of Health or other applicable online website(s). Receipts are proof of payment and are not acceptable as proof of renewal verification.  

Time and Attendance (HR-02-007)

How many tardy occurrences can I have before receiving disciplinary action?

Five (5) tardies within four (4) months can be subject to a corrective action.

How many times can I be absent (or call out) before receiving disciplinary action?

Three (3) attendance occurrences within six (6) months can be subject to a corrective action.

Is there a grace period before being considered tardy?

There will be a five (5) minute grace period before an occurrence is considered a tardy. If there is a pattern of abuse to the grace period, then management has the right to revoke the grace period.  

Note: The grace period does not automatically give an additional five minutes for when an employee is scheduled to start their shift.  This is provided to allow for situations outside of the control of the employee in getting to work on time (ex. A traffic accident, flat tire, inclement weather, etc.)

Holiday PL Bank and PL (HR-02-17)

How many holidays am I given during the fiscal year?

Ten (10)

What holidays are covered?

New Year’s Day, MLK Day, Memorial Day (non-premium), July 4th, Labor Day, Veteran’s Day, Thanksgiving Day, Day after Thanksgiving, Christmas Eve, Christmas Day.

How will I accrue the hours to cover those holidays?

Holiday hours will accrue within the pay period in which the holiday occurs.

What staff will accrue a holiday bank?

Nonexempt staff will accrue hours for the holiday within the pay period in which the holiday occurs. The total will depend upon the hours you work.

How does the holiday bank work?

Employees in areas who are closed in observance of company recognized holidays will be required to use PL from the holiday bank to cover that holiday:

  • Clinic and or administrative offices
  • Areas that are 24/7, and not closed in observance of the holiday, will be allowed to accrue the hours

What if I don’t use all of my PL holiday bank by the end of the fiscal year?

Employees will lose hours in the PL holiday bank if not used by the end of the fiscal year, June 30.

Other than holidays, what can I use my holiday PL bank for?

If not used, holiday bank PL hours can only be used for scheduled time off during the fiscal year. It cannot be used for unscheduled call outs.

Will UF Health continue to have PL sell back?

We will have one annual sell back of PL per year @ 75% of the amount the employee requests to sell back. Sell back is subject to business conditions and cash flow capabilities.

What happens to my PL if I terminate employment?

Upon termination, including resignation or discharge, not for cause, the employee will be paid for all accrued PL, based on years of service:

  • 0-3 years – 0%
  • 3-5 years – 50%
  • >5 years – 100%

What happens to my PL if I am terminated for cause?

Employees terminated for six (6) specific reasons, will forfeit all accrued PL:

  • Theft of hospital property, or property of other employees, patients, or visitors
  • If after an investigation, it is determined that an employee has engaged in fighting, physical, or verbal abuse of a patient, visitor, or other employee
  • Possession, use of, or threatening use of firearms or weapons, of any kind on hospital property
  • Falsification of time and attendance, payroll, or other hospital records
  • Reporting to work at any time under the influence of alcohol, or any controlled substance, including prescription medication, such that it creates a reasonable suspicion of behavioral and performance impairment (see Drug Free Workplace Policy (HR-01-003)
  • The unlawful possession, use, manufacture, sale, dispensation, distribution, or unauthorized administration of controlled substances, illegal drugs or alcohol on hospital premises at any time, or while conducting hospital business off the premises, is absolutely prohibited.

If I terminate my employment, are there situations or exceptions that would allow 100% PL payment outside of the years of service?

Yes, in the event that an employee separates due to the employee, spouse, or domestic partner of the military is being deployed, the employee will be entitled to payment of 100% of his/her PL balance, provided the appropriate deployment paperwork is provided.

When am I eligible to begin using my PL? 

Employees may not use personal leave during the first 90 days with the exception of the specific holidays designated by UF Health Jacksonville.

Probationary Period (HR-03-010)

How long is my probationary period?

The first six (6) months of employment with the organization an employee will remain in probationary status.

Can I request time off during my probation period?

Employees may not use personal leave during the first 90 days with the exception of the specific holidays designated by UF Health Jacksonville.

Can I transfer in my probation period?

During the probationary period you will not be eligible to transfer or apply for another job.

If I transfer do I have to go through another probation period?

Employees who are promoted/demoted, transferred, or involved in a lateral move to a new position (change of job class) are considered to be in an orientation status for six months.

Am I eligible for rehire if I leave within my probation period? 

An employee who terminates within their probationary period could result in being ineligible for rehire.
